Dedicated and compassionate Support Worker responsible for providing high-quality care and support to individuals with physical disabilities, learning disabilities, mental health conditions, autism, or other support needs. Committed to promoting independence, dignity, wellbeing, and social inclusion while ensuring a safe and supportive environment.

Key Responsibilities

* Deliver person-centred care and support in accordance with individual care plans.

* Assist service users with daily living activities, including personal care, dressing, bathing, meal preparation, and household tasks.

* Support individuals in developing life skills and maintaining independence.

* Accompany clients to appointments, educational activities, community events, and social engagements.

* Administer medication and maintain accurate medication records in line with organisational policies.

* Monitor and report changes in clients' physical, emotional, and mental wellbeing.

* Maintain accurate documentation, progress notes, and incident reports.

* Promote safeguarding, equality, diversity, and inclusion at all times.

* Provide emotional support and encouragement to service users and their families.

* Work collaboratively with healthcare professionals, social workers, and multidisciplinary teams.

* Ensure compliance with health and safety regulations, confidentiality requirements, and organisational procedures.
