How our sorting works

The order in which job vacancies are displayed is determined by a composite score based on the following factors:

  • Keyword Relevance: How well your search terms match the vacancy details. We prioritize matches found in the job title, followed by job requirements, location names, and educational levels. Matches within general employer information or the organization's name carry a lower weight.
  • Commercial Prioritization (Premium Jobs): Vacancies paid for by employers ('Premium' or 'Sponsored') receive a ranking boost and will appear higher in the search results.
  • Recency (Date Relevance): Newer vacancies are prioritized. The relevance score of a vacancy is reduced by half once the posting is older than 30 days.
  • Proximity (Distance Relevance): Vacancies located closer to your search location are ranked higher. For vacancies located more than 30 km from the search center, the relevance score is halved.

Single Homeless Project has an exciting opportunity for a Homeless Pathway Coordinator to join our team based across Newham, London. You will join us on a full-time, fixed term basis for 12 months. In return, you will receive a competitive salary starting at £34,127.11 and rising incrementally to £36,947.49 per annum (pro-rata).

About the Homeless Pathway Coordinator role:

No one should leave hospital with nowhere safe to go.

As Homeless Pathway Coordinator, you’ll be a consistent and trusted presence for patients experiencing homelessness, helping them navigate what can feel like an overwhelming mix of health, housing and support systems. You’ll spend time with patients on wards and in A&E, listening to what matters to them, understanding the barriers they face and making sure their voice remains at the heart of every decision.

Day to day, you’ll manage a caseload, complete assessments, develop clear discharge and support plans and make referrals to housing, healthcare, social care, mental health and substance use services. You’ll follow up actions, keep plans moving and advocate firmly when someone is at risk of being discharged without the right accommodation or support in place.

You’ll work closely with the Homeless Pathway Lead, hospital teams, Newham Council and SHP’s wider Health Team, bringing professionals together and helping everyone stay focused on the person behind the case. You’ll also build links with community services, identify gaps in existing pathways and share learning that helps the hospital respond more effectively to people experiencing homelessness.

This is a role where strong relationships can change the course of someone’s recovery. You’ll join an experienced and supportive team, with opportunities to deepen your knowledge of inclusion health, homelessness and psychologically informed practice through training, reflective practice and shared learning.

About you:

You have experience supporting people experiencing homelessness, multiple disadvantage or significant barriers to accessing services.

You know how to build trust with people who may feel let down by services, keeping their voice and choices at the centre of your work.

You can manage a caseload, assess what support is needed and turn complex situations into clear, practical next steps.

You’re a confident communicator and advocate who can bring health, housing and support services together around an individual.

You work calmly and compassionately, understand safeguarding and risk, and are keen to keep developing your knowledge through training and reflective practice.

Our attractive benefits package includes:

A salary increase after successfully completing six month's probationary period

A 37.5 hour working week including flexible working hours (core hours are 10am – 4pm) in non-accommodation services

25 days annual leave, increasing annually to the maximum 30 days (plus paid Bank Holidays),

A contributory pension scheme: Single Homeless Project will contribute the equivalent of 5% of your annual salary

Staff Health Cash Plan and discounts scheme

Comprehensive and integrated training programme designed specifically to develop the skills and knowledge involved in our work
