Job Title: Support Coordinator
Location: Southwark, London
Contract: 24 Weeks
Hours: 36 hours/week

About the Role

We are currently recruiting for an experienced and motivated Support Coordinator to join a team based at the Southwark Resource Centre, supporting adults with a range of needs including learning disabilities, autism, sensory needs, physical disabilities, dementia and age-related conditions.

This is an excellent opportunity for an experienced health and social care or SEND professional who is passionate about person-centred support, promoting independence and improving outcomes for the people they support.

Working closely with the Centre Manager and wider professional network, you will coordinate and lead a team delivering individualised, outcome-focused support across the resource centre, sister locations and community settings.

Key Responsibilities

* Lead, coordinate and support a team delivering high-quality, person-centred services.

* Promote independence, wellbeing, resilience and positive risk-taking.

* Develop and coordinate meaningful activities and opportunities for individuals to learn new skills and build relationships.

* Ensure support plans and Positive Behaviour Support (PBS) plans are implemented, reviewed and regularly updated.

* Provide effective supervision, line management, coaching and performance management to staff.

* Promote and maintain high standards of safeguarding, health and safety and risk management.

* Deputise for the Designated Safeguarding Lead when required.

* Ensure staff work in accordance with relevant policies, procedures and statutory frameworks.

* Support the development of SMART, person-centred outcomes and monitor progress against individual goals.

* Contribute to annual reviews and, where appropriate, the EHCP process.

* Work collaboratively with Adult Social Care, SEND professionals, NHS colleagues, community services and other partner organisations.

* Promote equality, diversity and inclusion and challenge discriminatory practice.

* Maintain accurate records, reports and service data.

* Support the Centre Manager with service delivery, expenditure monitoring and operational management when required.

* Help ensure activities delivered within the centre and wider community are safe, meaningful and outcome-focused

About You

We are looking for someone who has strong leadership skills and a genuine commitment to supporting people with disabilities and/or SEND to achieve greater independence.

You will ideally have:

* NVQ Level 5 in Health & Social Care, or a relevant SEND teaching qualification such as HLA/SEND teaching/QTS, or a willingness to undertake the qualification.

* At least 2 years' experience working in an equivalent setting, supporting people with disabilities and/or SEND.

* Good knowledge of safeguarding, Health & Safety, Mental Capacity Act (MCA), Deprivation of Liberty Safeguards (DoLS) and Positive Behaviour Support (PBS).

* Experience developing person-centred support plans and SMART outcome-based goals.

* Experience contributing to and implementing PBS plans.

* Experience preparing for and leading annual reviews and/or contributing to the EHCP process.

* Knowledge or experience of alternative communication systems.

* Strong leadership, communication, organisation and problem-solving skills.

* Experience supervising and managing staff, including performance management and supporting professional development.

* Experience planning and coordinating safe, positive activities within community settings.

* Excellent written, numerical and IT skills, with the ability to maintain accurate records and produce reports.

* A strong understanding of dignity, respect, equality and person-centred practice.

* The ability to support or provide guidance around functional activities such as personal care, transfers, eating and drinking, where required.
