We are looking for an organised and proactive Care Coordinator to join our client’s friendly and dedicated team based at their Dereham office. This is a rewarding opportunity to play a key role in ensuring the smooth delivery of high-quality, person-centred care to individuals with complex healthcare needs in the community.
About the Role
As a Care Coordinator, you will be responsible for planning and coordinating care packages, ensuring that the individuals we support receive consistent, safe, and effective care. You will work closely with our nursing team, support workers, service users, and external professionals to maintain excellent standards across all services.

Key Responsibilities
Coordinate and manage care packages to meet individual needs
Create and maintain staff rotas, ensuring continuity of care
Allocate support workers to assignments based on skills and availability
Act as a point of contact for service users, families, and staff
Support the onboarding of new care packages
Liaise with healthcare professionals, commissioners, and internal teams
Monitor service delivery and respond to any issues or changes in care needs
Maintain accurate records and ensure compliance with company policies and regulatory standards
Support recruitment, onboarding, and ongoing supervision of support staff
What They are Looking For
Previous experience in a care coordination, scheduling, or similar role within health or social care
Strong organisational and time management skills
Excellent communication and interpersonal abilities
Ability to work under pressure and manage multiple priorities
A good understanding of person-centred care
IT literacy and confidence using scheduling systems
A proactive and solution-focused approach
Desirable Skills & Experience
Experience working with individuals with complex care needs
Knowledge of community care services
Experience supporting or supervising care staff
