How our sorting works

The order in which job vacancies are displayed is determined by a composite score based on the following factors:

  • Keyword Relevance: How well your search terms match the vacancy details. We prioritize matches found in the job title, followed by job requirements, location names, and educational levels. Matches within general employer information or the organization's name carry a lower weight.

Discharge & Recovery Coordinator - Housing, Health & Wellbeing Hospital Discharge Support Service (2 positions available)

Location: Severn House, Belmont, County Durham, DH1 1TH
Working Hours: 37 hours per week
Contract Type: Permanent
Salary: £25,877.80 - £34,240 per annum

About the role

We're looking for compassionate and motivated Discharge & Recovery Coordinators to join Waythrough as we mobilise the Housing, Health & Wellbeing Hospital Discharge Support Service across County Durham.

This is an exciting opportunity to support adults experiencing homelessness or at risk of homelessness at a crucial point in their lives. Working closely with hospital teams, Adult Social Care, housing providers and other partner agencies, you'll help ensure people can leave hospital safely with the right accommodation, support and recovery plans in place.

You'll provide person-centred support both within hospital settings and in the community, managing your own caseload and working collaboratively with a range of professionals to coordinate safe and sustainable hospital discharges. Through comprehensive assessments, support planning and ongoing recovery-focused interventions, you'll empower people to overcome barriers, improve their wellbeing and reduce the risk of homelessness and hospital readmission.

You'll also work closely with local housing authorities and accommodation providers to identify appropriate housing options, complete Duty to Refer notifications where required and ensure individuals receive timely access to the services and support they need.

About the service

The Housing, Health & Wellbeing Hospital Discharge Support Service aims to prevent individuals aged 18 and over from being discharged from hospital into street homelessness, while supporting safe, timely and sustainable discharges through coordinated in-hospital intervention and time-limited post-discharge support.

The service provides proactive, responsive and coordinated support through:

* Early identification of individuals who are homeless or at risk of homelessness.

* In-hospital assessment, advocacy and discharge coordination.

* Time-limited post-discharge support to help people sustain accommodation and reduce the risk of hospital readmission.

The service specifically supports individuals who:

* Are homeless or at risk of homelessness.

* Have support needs relating to drug and alcohol use.

* Are known to Adult Social Care services, including those with assessed care and support needs, those open to Adult Social Care teams, and those requiring coordinated discharge planning.

About you

We're looking for someone who is passionate about supporting people to achieve positive and lasting change.

You'll have experience working with people who have complex or multiple support needs and be confident building positive relationships with individuals and partner organisations. You'll be able to manage your own caseload, complete high-quality assessments and work collaboratively with a range of professionals to coordinate safe hospital discharges and improve outcomes for the people you support.

You'll also have:

* A Level 2 qualification in Health & Social Care or a related subject.

* Knowledge of housing options and relevant housing legislation.

* Experience or understanding of support planning, needs assessments and risk management.

* Excellent communication and interpersonal skills, with the ability to build effective relationships with individuals and partner agencies.

* The ability to maintain accurate records and use electronic case management systems.

* A good understanding of safeguarding and person-centred practice.

* A full UK driving licence and access to your own vehicle, as regular travel across County Durham is an essential requirement of the role.

Experience of working within hospital discharge, homelessness, housing, health, social care or substance use services would be advantageous but is not essential.
