How our sorting works

The order in which job vacancies are displayed is determined by a composite score based on the following factors:

Job description

Support Coordinator
We’re looking for an innovative, passionate and professional individual with excellent communication and organisational skills to join the Life After Stroke Service on the Isle of Wight.
Position: (phone number removed) Stroke Association Support Coordinator
Location: Home-based, Isle of Wight. However, Frequent travel will be required as part of this role (to also include team meetings or other work-related meetings on the mainland)
Hours: Part-time, 21 hours per week
Salary: Circa £17,000 per annum (FTE circa £28,300 per annum)
Contract: This is a fixed-term contract until 31st March 2027. Our services are contracted; we currently have funding for this contract until 31st March 2027

The Role
This is an exciting opportunity to work with stroke survivors and their families to support them following a stroke.
The service aims to identify and provide key worker support to meet the needs of stroke survivors and carers across the stroke pathway. Providing a range of innovative support solutions, supporting them to meet their desired outcomes.
The Stroke Association Support Coordinator will:
* Support new stroke survivors and their carers from hospital discharge into the community.
* Provide personalised information, advice and support.
* Support stroke survivors to make informed lifestyle changes which will help them to prevent further strokes.
* Work collaboratively with NHS colleagues and other areas of the community to make a difference in the lives of people affected by stroke.
About you:
You will:
* Have a background in a caring and/or charity profession supporting people with disabilities.
* Be able to demonstrate previous experience of working collaboratively with other organisations to achieve joint ambitions.
* Have experience of working with people who may have additional communication support needs.
* Be proficient in using Microsoft applications, including Word and Excel and experience using IT systems to record and maintain beneficiary data.
This role requires extensive travel across the Isle of Wight to visit people at home and in community settings. Travel to the mainland will also be required for regular team meetings and other work-related meetings. Candidates must be able to demonstrate how they can meet this requirement of the role.
To fulfil the role, you must be a resident of the UK and have the right to work in the UK.
