How our sorting works

The order in which job vacancies are displayed is determined by a composite score based on the following factors:

  • Keyword Relevance: How well your search terms match the vacancy details. We prioritize matches found in the job title, followed by job requirements, location names, and educational levels. Matches within general employer information or the organization's name carry a lower weight.
  • Commercial Prioritization (Premium Jobs): Vacancies paid for by employers ('Premium' or 'Sponsored') receive a ranking boost and will appear higher in the search results.
  • Recency (Date Relevance): Newer vacancies are prioritized. The relevance score of a vacancy is reduced by half once the posting is older than 30 days.
  • Proximity (Distance Relevance): Vacancies located closer to your search location are ranked higher. For vacancies located more than 30 km from the search center, the relevance score is halved.
The final ranking is established by multiplying all these individual factors to calculate the total relevance score.

    Job description

Looking for a role where you can make a real difference while enjoying genuine flexibility and support?

This Discharge to Assess (D2A) Social Worker opportunity offers a hybrid working model, with the flexibility to work from home for part of the week while remaining closely connected to hospital and community teams. You'll join a local authority that values collaboration, professional development, and employee wellbeing.

Working within a fast-paced multidisciplinary environment, you'll play a vital role in supporting people to leave hospital safely and receive ongoing assessment in the most appropriate setting, promoting independence and achieving positive outcomes.

    Benefits include:

Local Government Pension Scheme (LGPS) – A secure, defined benefit pension providing long-term financial security.
Excellent annual leave – 27–33 days plus bank holidays, giving you the opportunity to maintain a healthy work-life balance.
Hybrid and flexible working – A mix of hospital, community, and home-based working to help you manage your workload effectively.
Stable public sector employment – Enjoy long-term career security within a supportive local authority.
Career development opportunities – Clear progression routes into Senior Practitioner, Advanced Practitioner, and management positions, alongside ongoing training and learning.
    The Role

We're recruiting experienced Discharge to Assess Social Workers to join a busy and supportive team focused on delivering safe, timely hospital discharges.

Working closely with health professionals, therapists, ward staff, and community partners, you'll complete strengths-based assessments following discharge, coordinate care and support, and ensure individuals receive the right services to maximise independence and reduce unnecessary readmissions.

    You'll be responsible for:

Completing Care Act assessments within the Discharge to Assess pathway.
Supporting safe and timely discharges from acute hospital settings.
Undertaking Mental Capacity Assessments and Best Interest decisions where required.
Coordinating care packages and community-based support.
    About You

    To be considered, you'll need:

    A recognised Social Work qualification.
    Current Social Work England registration.
Experience working within Hospital Discharge, Discharge to Assess, or Adult Community Services.
A sound understanding of the Care Act, Mental Capacity Act, and strengths-based practice.
Experience using Mosaic or a similar case management system is desirable.
If this sounds like the next step in your career, we'd love to hear from you.
