How our sorting works

The order in which job vacancies are displayed is determined by a composite score based on the following factors:

  • Keyword Relevance: How well your search terms match the vacancy details. We prioritize matches found in the job title, followed by job requirements, location names, and educational levels. Matches within general employer information or the organization's name carry a lower weight.
  • Commercial Prioritization (Premium Jobs): Vacancies paid for by employers ('Premium' or 'Sponsored') receive a ranking boost and will appear higher in the search results.
  • Recency (Date Relevance): Newer vacancies are prioritized. The relevance score of a vacancy is reduced by half once the posting is older than 30 days.
  • Proximity (Distance Relevance): Vacancies located closer to your search location are ranked higher. For vacancies located more than 30 km from the search center, the relevance score is halved.
The final ranking is established by multiplying all these individual factors to calculate the total relevance score.

    Job description

Halton Borough Council is seeking an experienced, enthusiastic and committed Social Worker to join its Hospital Discharge Team, based at Whiston Hospital in Prescot.

The Hospital Discharge Team is an adult social work service responsible for completing timely, strengths-based assessments for adults aged 18 and over. The team works collaboratively with health and social care professionals to facilitate safe, effective and timely hospital discharges, ensuring that individuals receive the right support to maintain their independence and wellbeing.

    Key Responsibilities

Complete strengths-based assessments and develop personalised support plans to facilitate safe and timely hospital discharges.

Work with adults with a range of needs, including older people, adults with learning disabilities, and adults with physical and sensory impairments.

Promote independence, choice, dignity and wellbeing, ensuring that individuals remain at the centre of all decisions about their care and support.

Undertake risk assessments and contribute to effective risk management to support safe discharge planning.

Apply relevant legislation, including the Care Act 2014 and the Mental Capacity Act 2005, to inform assessments and decision-making.

Work effectively with multidisciplinary and multi-agency professionals within the hospital setting.

Manage and prioritise your own caseload, ensuring assessments, support plans and discharge arrangements are completed within appropriate timescales.

Challenge discrimination and promote equality, inclusion and person-centred practice.

    Essential Skills and Experience

    The successful candidate will:

Have a sound knowledge of relevant social work legislation, particularly the Care Act 2014 and Mental Capacity Act 2005, and a clear understanding of their principles.

Ideally have experience in undertaking risk assessments and implementing effective risk management strategies.

Demonstrate a strong commitment to strengths-based, person-centred social work practice.

Have excellent communication, assessment and decision-making skills.

Be confident working collaboratively with multidisciplinary teams and partner agencies in a fast-paced hospital environment.

Be able to manage and organise their own caseload effectively, balancing competing priorities and meeting deadlines.

Be passionate about promoting independence, challenging discrimination and achieving positive outcomes for adults with care and support needs.

    Why Join the Team?

This is an opportunity to make a meaningful difference to adults and their families at a crucial point in their lives. You will play an important role in supporting safe and timely hospital discharges, promoting independence and ensuring that people receive appropriate, personalised care and support.
