Looking for an experienced Social Worker to support a focused project reviewing overdue out-of-borough placements ahead of an upcoming CQC inspection.
This is a great opportunity for a Social Worker with strong assessment and review skills to ensure individuals are receiving the right care and support to meet their needs.
Key Responsibilities:
š¹ Review out-of-borough placements that have not been reviewed in the past 2ā3 years.
š¹ Visit service users in their current placements and arrange meetings with family members where appropriate.
š¹ Complete reviews of care and support plans to ensure needs are being met.
š¹ Undertake reassessments where there has been a significant change in an individual's needs.
š¹ Work collaboratively with service users, families and care providers to achieve positive outcomes.
š¹ Aim to complete approximately 5ā7 reviews per week.
Requirements:
ā Qualified Social Worker registered with Social Work England.
ā Experience completing statutory reviews and assessments.
ā Confident working independently and managing a varied workload.
ā Strong communication and organisational skills.
