Responsibilities
Case Management: Manage a caseload of vulnerable residents with complex personal needs, mental health issues, or safeguarding concerns.
Assessments & Support Plans: Conduct home visits and holistic assessments to draft, monitor, and review personalized support action plans.
Income Maximization & Benefits: Provide specialist guidance on Universal Credit, Housing Benefit, Discretionary Housing Payments, and debt management.
Safeguarding & Welfare: Perform regular welfare checks, identify risks of domestic abuse or exploitation, and submit appropriate safeguarding referrals.
Multi-Agency Working: Build and maintain networks with health services, social care, employment training providers, and voluntary organizations.
