How our sorting works

The order in which job vacancies are displayed is determined by a composite score based on the following factors:

  • Keyword Relevance: How well your search terms match the vacancy details. We prioritize matches found in the job title, followed by job requirements, location names, and educational levels. Matches within general employer information or the organization's name carry a lower weight.
  • Commercial Prioritization (Premium Jobs): Vacancies paid for by employers ('Premium' or 'Sponsored') receive a ranking boost and will appear higher in the search results.
  • Recency (Date Relevance): Newer vacancies are prioritized. The relevance score of a vacancy is reduced by half once the posting is older than 30 days.
  • Proximity (Distance Relevance): Vacancies located closer to your search location are ranked higher. For vacancies located more than 30 km from the search center, the relevance score is halved.
The final ranking is established by multiplying all these individual factors to calculate the total relevance score.

    The starting salary for this position is £43,633 per annum based on a 36-hour (full-time) working week.

    Transition are a specialist Adult Social Care team supporting young adults as they move into adulthood. We work closely with young people aged from 14, and their carers or families.

    The Transition Social Worker role plays a critical part in delivering the organisation's strategic priorities for young people with physical disabilities, learning disabilities and autism moving into adulthood; translating vision into meaningful, person-centred outcomes.

    Operating at the intersection of policy, practice, and partnership, you will help drive improvements in service quality, early intervention, and integrated working, ensuring that individuals and their carers receive the right support at the right time.

    The successful candidate will have a strong opportunity to influence how services evolve, contributing to innovation and continuous improvement across both Adults' and Children's Social Care. By building effective relationships with colleagues, partners, and local communities, you will help shape more responsive, inclusive, and sustainable approaches that align with our long-term aspirations.

    This role offers significant scope to make a tangible difference, enhancing the experiences of young people, while reducing inequalities and ensuring resources are used effectively. Through professional leadership, insight, and a commitment to best practice and sound budget management, you will help deliver measurable impact at both operational and strategic levels.

    Your key responsibilities as a Social Worker in the Transition Team will include:

    Managing and prioritising a case load
    Legal court work
    Working closely with partners
    Completing Section 42 enquiries
    Working on duty
    Supporting the team with professional development
    Supervising Senior Social Care Assistants to ensure a high-quality standard of service is providedOur team offers a positive, supportive working environment where training and development is prioritised.

    Your Application

    In order to be considered for shortlisting, your application will clearly evidence the following skills and align with our behaviours:

    Professional qualification in Social Work and current registration with Social Work England
    Successful completion of an Assessed and Supported Year in Employment (ASYE) programme if qualified as a social worker after June 2012
    Experience in Social Work, including assessment and risk management skills
    Commitment to your own professional development, including maintaining up-to-date knowledge of practice issuesAn enhanced DBS 'Disclosure and Barring Service' check for regulated activity (formerly known as CRB) and the Adults' Barred List checks will be required for this role.

    Surrey has both urban and rural areas and candidates will be expected to have a valid driving licence to drive in the UK, access to a vehicle and be willing to travel across a wide geographical area.     Local Government Reorganisation (LGR)

    Surrey County Council is undergoing Local Government Reorganisation and will move to new authorities in April 2027. This role is currently employed by Surrey County Council and will remain so until then. If you are employed by Surrey County Council on 1 April 2027, your employment will transfer under TUPE arrangements to one of the new councils, with your current terms and conditions protected.

    The provisional allocation for this post is East Surrey Council, subject to final confirmation through the wider transition process later this year.
