Job Title: Support Mentor
Location: Royal Borough of Kensington and Chelsea (RBKC) - Unfortunately this location does not have step free access
Salary: £28,800
Shift Pattern: 37.5 hours per week Monday to Friday 09:00 - 17:00. You will work on a rota and may be required to work outside these hours as per service, rota, and resident requirements.
About the Role
We're hiring a Support Mentor to join our team based in Kensington and Chelsea. In this role, you will support residents who reside in our supported accommodation and have experience of homelessness and have multiple disadvantages such as with their mental health, substance use, or have an offending history.
As a Support Mentor, you will identify individual resident needs, and provide person centred, tailored support to residents to help them overcome their personal challenges and achieve their goals. You will be part of a supportive, collaborative team which delivers person-centered support to our residents, to enable them to build independence, improve wellbeing, and move on to longer-term housing. You will do this through a combination of producing tailored support plans, activities, and partnership working. The service operates across several locations in Kensington and Chelsea, with good transport links and access to ongoing training, development and career progression opportunities.
Key Responsibilities Include:
* Support residents who may be facing challenges with homelessness, mental health, substance use, or feeling independent
* Create tailored support plans through one to one sessions, group sessions, and multi agency meetings
* Support residents to build skills and confidence to support move on to long-term independent living
* Monitor residents’ wellbeing, carrying out regular welfare checks, and responding to any incidents or emergencies.
* Plan and deliver activities to engage residents with other people, communities, and opportunities. This may include days out the service and in service activities
* Keep support plans updated on our online portals as appropriate
About You
We're looking for a passionate individual who understands the needs of our residents including [list their needs]. You will be a self motivated individual who is able to be proactive in managing your own time during busy or quiet periods to ensure productivity. You will be able to work collaboratively as part of a team, and will have alignment with our SIG values. You will be keen to support our residents to achieve their individual goals and support them to be the best versions of themselves.
* Previous professional or lived experience supporting people who have multiple and complex needs
* Ability to engage with people from different backgrounds, build rapport, and empower them to achieve their personal goals
* Ability to show empathy and compassion and the different challenges people face; ability to come up with creative solutions to support them in their journey
* Ability to use, learn and adapt to IT at an intermediate level, including Microsoft and other software programs
* Ability and willingness to show flexibility of working patterns, responding to the needs of the service and residents
