How our sorting works

The order in which job vacancies are displayed is determined by a composite score based on the following factors:

  • Keyword Relevance: How well your search terms match the vacancy details. We prioritize matches found in the job title, followed by job requirements, location names, and educational levels. Matches within general employer information or the organization's name carry a lower weight.
  • Commercial Prioritization (Premium Jobs): Vacancies paid for by employers ('Premium' or 'Sponsored') receive a ranking boost and will appear higher in the search results.
  • Recency (Date Relevance): Newer vacancies are prioritized. The relevance score of a vacancy is reduced by half once the posting is older than 30 days.
  • Proximity (Distance Relevance): Vacancies located closer to your search location are ranked higher. For vacancies located more than 30 km from the search center, the relevance score is halved.
The final ranking is established by multiplying all these individual factors to calculate the total relevance score.

    About Us

    At 360 Community Care, we provide high-quality domiciliary care to people who require support to live independently in their own homes and communities.

    About the Role

    We are looking for a Care Coordinator to join our team and help ensure our care services are safe, well-coordinated and consistently delivered to a high standard.

    You will work closely with the people we support, their families, care workers, healthcare professionals, commissioners and the Registered Manager. You will play an important role in coordinating care packages, supporting staff and maintaining excellent standards of care and compliance.

    This role would suit someone who is organised, compassionate, confident and passionate about making a positive difference to people's lives.

    Key Duties

    Coordinate care packages and allocate care workers according to individual needs.

    Maintain accurate care schedules, care plans and service documentation.

    Work with PWS, families and professionals to develop and review person-centred care plans.

    Respond to staff absences and emergencies, ensuring continuity of care.

    Support care workers with guidance, communication and day-to-day queries.

    Monitor care quality, safeguarding, risk management and CQC compliance.

    Attend reviews, safeguarding meetings and other relevant meetings.

    Support audits, quality assurance and client feedback processes.

    Build positive relationships with PWS, families, staff and external stakeholders.

    Monitor service KPIs and ensure high standards of care delivery are maintained.

    Support recruitment, induction and training of care staff.

    Participate in the on-call rota.

    Essential Requirements

    Previous experience within healthcare, social care or domiciliary care industry as either a Care Coordinator or Recruiter.

    Experience of coordinating or supervising care services is desirable.

    Good understanding of person-centred care, safeguarding and risk management.

    Strong organisational and communication skills.

    Ability to work independently and manage competing priorities.

    Ability to build positive relationships with PWS, families and professionals.

    Good understanding of confidentiality, record keeping and regulatory requirements.

    Things to Note:

    This role involves direct access to the people we support.

    A full Enhanced DBS check will be required.

    On-call cover is required as part of the role.

    Full induction and clinical training will be provided.

    Fully funded Care Certificate training and safeguarding training are available.
