About the Role
As part of the Adult Social Care Assessment & Duty Team, you will play a key role in responding to referrals, assessing the needs of adults and helping individuals maintain their independence and wellbeing.
You will work with adults with a range of care and support needs, providing timely interventions, completing assessments and coordinating services to achieve positive outcomes.
Key Responsibilities

Complete Care Act assessments, reviews and support planning.
*
Respond to duty referrals and identify appropriate support pathways.
*
Work with adults, families, carers and partner agencies to understand individual needs.
*
Provide information, advice and guidance to promote independence and choice.
*
Assess risks and contribute to safeguarding decisions where required.
*
Work collaboratively with health professionals and wider community services.
*
Maintain accurate case records and complete statutory documentation.
*
Support strengths-based practice by focusing on what matters most to each individual.
What We’re Looking For
*
Qualified Social Worker with current Social Work England registration.
*
Strong Adult Social Care experience.
*
Experience within assessment, duty or community teams is highly desirable.
*
Good knowledge of the Care Act 2014, Mental Capacity Act 2005 and safeguarding procedures.
*
Ability to manage a varied caseload and make confident professional decisions.
