Key Responsibilities
*
Manage a caseload using a strengths-based approach to support adults in achieving greater independence.
*
Complete assessments, reviews and support plans.
*
Work closely with residents, carers, health professionals and partner agencies to deliver positive outcomes.
*
Coordinate short-term care and support carers to help prevent breakdown.
*
Participate in safeguarding, risk assessments and protection planning.
*
Maintain accurate case records and prepare documentation where required.
*
Work collaboratively within a Multi-Disciplinary Team to deliver person-centred services.
Requirements
*
Qualified Social Worker with current Social Work England registration.
*
Adult Social Work experience with strong assessment and care planning skills.
*
Good knowledge of the Care Act, safeguarding and strengths-based practice.
*
Ability to work effectively within a multi-agency environment.
