How our sorting works

The order in which job vacancies are displayed is determined by a composite score based on the following factors:

  • Keyword Relevance: How well your search terms match the vacancy details. We prioritize matches found in the job title, followed by job requirements, location names, and educational levels. Matches within general employer information or the organization's name carry a lower weight.
  • Commercial Prioritization (Premium Jobs): Vacancies paid for by employers ('Premium' or 'Sponsored') receive a ranking boost and will appear higher in the search results.
  • Recency (Date Relevance): Newer vacancies are prioritized. The relevance score of a vacancy is reduced by half once the posting is older than 30 days.
  • Proximity (Distance Relevance): Vacancies located closer to your search location are ranked higher. For vacancies located more than 30 km from the search center, the relevance score is halved.
The final ranking is established by multiplying all these individual factors to calculate the total relevance score.

    Job Title: Care Coordinator

    Reporting to: Head of Service & Operations / Registered Care Manager

    Salary: £26,000 - £30,000 depending on experience

    Location: Battersea, London (Office-based role)

    Hours: Full time, with participation in the on-call rota as required.

    About the Role

    The Care Coordinator is a full-time, on-site role based in our Battersea office. This position is responsible for coordinating complex and domestic care services, ensuring that care plans are accurately developed, implemented, and regularly reviewed. Day-to-day tasks include liaising with clients, families, case managers and healthcare professionals, scheduling care staff, and matching support workers to client needs. The role also involves monitoring service quality, handling queries and concerns, maintaining detailed records, and supporting smooth communication between all parties. The Care Coordinator will contribute to continuous improvement of care processes and uphold Handling Care's standards of compassion, trust, and person-centred care.


    Key Responsibilities


    Care Coordination

    • Coordinate and manage care packages to ensure all visits are covered.
    • Produce and maintain staff rotas.
    • Arrange emergency cover for sickness, annual leave and unexpected absences.
    • Match carers with clients based on skills, experience, location and compatibility.
    • Monitor electronic care records and ensure care is delivered as planned.
    • Ensure continuity and consistency of care wherever possible.

    Client Management

    • Build and maintain positive relationships with clients and their families.
    • Liaise with case managers, social workers, occupational therapists, commissioners and healthcare professionals.
    • Assist with new client onboarding and service implementation.
    • Monitor client satisfaction and respond to concerns promptly.
    • Support with care reviews and changes to care packages.

    Staff Management

    • Maintain regular communication with care staff.
    • Support staff with rota queries and scheduling issues.
    • Monitor staff availability and annual leave.
    • Assist with recruitment, onboarding and induction where required.
    • Ensure staff compliance with mandatory training and documentation.
    • Participate in staff supervisions and competency monitoring where appropriate.

    Compliance & Quality

    • Ensure care delivery complies with CQC regulations and company policies.
    • Maintain accurate electronic records.
    • Monitor incidents, accidents and safeguarding concerns.
    • Escalate concerns appropriately.
    • Support audits and quality assurance processes.
    • Assist in maintaining excellent service standards.

    Administration

    • Update care management systems.
    • Maintain accurate client and staff records.
    • Process referrals and new enquiries.
    • Monitor service capacity.
    • Produce reports as requested by management.

    On-Call Duties

    • Participate in the out-of-hours on-call rota.
    • Respond to emergencies.
    • Arrange emergency staffing cover.
    • Support care staff outside office hours.


    Who we're looking for


    Essential Skills & Experience

    • Demonstrated ability in Care Coordination and Case Management, including developing, reviewing, and updating care plans.
    • Strong Communication skills, with the capacity to collaborate effectively with clients, families, and multidisciplinary teams.
    • Knowledge of safeguarding, confidentiality standards, and regulatory requirements in care services.
    • Organisational and time-management skills, including scheduling, record-keeping, and prioritising multiple tasks.
    • Previous experience in a coordination or supervisory role within domiciliary care is an advantage.
    • Excellent Customer Service skills, with a focus on responsiveness, empathy, and problem-solving.
    • Confident using care management software and Microsoft Office.
    • Knowledge of CQC Fundamental Standards.
    • Full UK driving licence and access to a vehicle (preferred).

    Desirable

    • NVQ Level 3 in Health & Social Care (minimum).
    • Level 5 qualification (or working towards one).
    • Experience working with private clients, case management and complex care.
