The Role
As part of a collaborative integrated Mental Health and Wellness Team, you will:

Complete Care Act Assessments and statutory reviews.
*
Undertake safeguarding enquiries and interventions.
*
Provide care coordination under the CPA Framework.
*
Manage a caseload of approximately 25 service users.
*
Participate in the team duty rota, including screening, triaging, and managing new referrals.
*
Work closely with multidisciplinary professionals to deliver high-quality, person-centred mental health services.
Requirements
*
Qualified Social Worker with current Social Work England registration.
*
Experience within Adult Mental Health Services.
*
Strong knowledge of the Care Act, safeguarding legislation, and the CPA Framework.
*
Excellent assessment, communication, and case management skills.
*
Ability to work effectively within a multidisciplinary team.
