How our sorting works

The order in which job vacancies are displayed is determined by a composite score based on the following factors:

  • Keyword Relevance: How well your search terms match the vacancy details. We prioritize matches found in the job title, followed by job requirements, location names, and educational levels. Matches within general employer information or the organization's name carry a lower weight.
  • Commercial Prioritization (Premium Jobs): Vacancies paid for by employers ('Premium' or 'Sponsored') receive a ranking boost and will appear higher in the search results.
  • Recency (Date Relevance): Newer vacancies are prioritized. The relevance score of a vacancy is reduced by half once the posting is older than 30 days.
  • Proximity (Distance Relevance): Vacancies located closer to your search location are ranked higher. For vacancies located more than 30 km from the search center, the relevance score is halved.
The final ranking is established by multiplying all these individual factors to calculate the total relevance score.

    Job description

    Southwark Council are looking for an Head of Age Well for managing NHS Continuing Health Care (CHC) team and Better Care Fund (BCF) initiatives including developing proposals for BCF and reporting the NHS England.

    £425 per day - Please note the rate for this role is set my NHS Agenda for Change and is not negotiable at £425 per day Umbrella Pay Rate.

    Hybrid Working usually 2-3 days in the office but expected to be flexible for external meetings.

    This post is joint funded by the NHS, it is essential that candidates have experience of working in the NHS or an integrated commissioning function. It is essential that candidates have experience of managing NHS Continuing Health Care (CHC) team and Better Care Fund (BCF) initiatives including developing proposals for BCF and reporting the NHS England.

    Job Description:

    This role is a key part of the commissioning team which is funded by Southwark Council and SEL ICS. The role leads a commissioning team focused on older and/or frail residents, including physical disabilities, of Southwark.

    The post holder will have a significant role in building working relationships and collaboration across the council, the wider health system and through Partnership Southwark.

    As part of this team the post holder will lead and therefore significantly contribute to developing and delivering a specified programme of work which will focus on a defined population within Southwark.

    The post holder will be expected to contribute to the professional development of the team and co-ordinate the delivery of a complex programme of work centred on core health and care priorities in Southwark and South East London.

    The post holder will also significantly contribute to planning for continuous reduction in health and wellbeing inequalities across the borough in collaboration across the council, the wider health system and through Partnership Southwark. Working across the division, the post holder will take stock of the current commissioning arrangements and the status of public health-commissioned services, engaging with Public Health Consultants, Heads of Programme, and Policy Officers to develop and implement a commissioning plan for the division.

    The post holder should assess, rationalise, and deliver all aspects of high quality public health commissioning with a view to tackling inequalities, improving outcomes and generating value for Southwark residents.

    The post holder will be responsible for creating and sustaining good working relationships in support of commissioning key public health services and their links with other related services, within the Council, the NHS, voluntary sector and other relevant local partners, as well as commissioned service providers.

    The post holder will be required to attend occasional evening and weekend meetings and events in a variety of settings and locations across the borough, in support of the delivery of their work programme.

    The post holder reports to the Assistant Director for Integrated Commissioning
