How our sorting works

The order in which job vacancies are displayed is determined by a composite score based on the following factors:

  • Keyword Relevance: How well your search terms match the vacancy details. We prioritize matches found in the job title, followed by job requirements, location names, and educational levels. Matches within general employer information or the organization's name carry a lower weight.
  • Commercial Prioritization (Premium Jobs): Vacancies paid for by employers ('Premium' or 'Sponsored') receive a ranking boost and will appear higher in the search results.
  • Recency (Date Relevance): Newer vacancies are prioritized. The relevance score of a vacancy is reduced by half once the posting is older than 30 days.
  • Proximity (Distance Relevance): Vacancies located closer to your search location are ranked higher. For vacancies located more than 30 km from the search center, the relevance score is halved.
The final ranking is established by multiplying all these individual factors to calculate the total relevance score.

    Job description

    Package Description: Practice Nurse

    The Role

Practice nursing is where clinical expertise, continuity of care and prevention come together.

It’s where you support patients to stay well, manage long-term conditions, and access the right care at the right time - often building long-term relationships that make a real difference to people’s health and wellbeing.

At IC24, we’re looking for an experienced Practice Nurse to join our Primary Care team across Allied Medical Practice and Ship Street Surgery in Brighton. This is a varied and rewarding role where you’ll use your full range of nursing skills to deliver high-quality, patient-centred care within a supportive multidisciplinary team.

You’ll work across a busy primary care environment, delivering everything from chronic disease management and immunisations to health screening and minor treatments. You’ll also play an important role in prevention, early intervention and health promotion across the local population.

At IC24, primary care is not just reactive - it’s connected.

You’ll work within an integrated healthcare system designed to improve access, reduce pressure on secondary care services and ensure patients receive the right care, in the right place, at the right time.

    What you’ll focus on

    → Delivering high-quality nursing care

You’ll provide assessment, treatment, screening, health education services and advice to patients, working in accordance with the NMC Code of Conduct and practice protocols and guidelines.

    → Managing long-term conditions

You’ll diagnose, monitor and support the development of individual management plans for patients with conditions such as diabetes, respiratory disease and cardiovascular disease, working in partnership with patients and other healthcare professionals.

→ Prevention, screening and health promotion

You’ll undertake NHS Health Checks, cervical screening, immunisations, travel health advice, smoking cessation support and wider preventative health interventions.

→ Clinical procedures and treatment delivery

You’ll carry out wound care, ear care, phlebotomy, ECGs, blood pressure monitoring and other routine clinical procedures safely and in line with practice policy.

→ Supporting quality and clinical governance

You’ll ensure accurate coding and recording of patient data to support QOF and Enhanced Services and contribute to high standards of clinical governance and best practice.

→ Working within a multidisciplinary team

You’ll work both independently and in participation with GPs, HCAs, General Practice Assistants and wider clinical colleagues to deliver safe, effective and coordinated patient care.

    → Risk assessment and safeguarding

You’ll recognise safeguarding concerns and wider risk issues (including domestic abuse, child protection and vulnerable adults), ensuring appropriate escalation in line with policy.

    What matters in this role

We’re not looking for one type of Practice Nurse - but there are some things that matter.

    You’re likely someone who:

    • Is a Registered Nurse with current NMC registration
     • Has experience working in general practice or primary care
     • Has experience managing long-term conditions
    • Demonstrates strong clinical judgement and safe decision-making
     • Communicates clearly and effectively with patients and colleagues
    • Is confident working independently and as part of a team
     • Has strong organisational skills and attention to detail in clinical documentation
    • Is committed to delivering compassionate, patient-centred care

Most importantly, you’ll be motivated by delivering high-quality care that improves outcomes for patients every day.

    Who are we?

We are Integrated Care 24 (IC24), the leading not-for-profit Social Enterprise providing innovative and patient focused primary care services. IC24 is committed to improving access to health and social care for our patients and reducing the demand on secondary care services. IC24 provides services to over 6 million patients, including GP-led out-of-hours services, NHS 111, primary care and secondary care support services.

    Location

    Ship Street Surgery, Brighton

    • Ship Street Surgery, Ground Floor/First Floor, Ship Street, Brighton, BN1 1AE

    What’s in it for you?

    • Annual pay of £43,663.40
     • Opportunity to join the NHS Pension Scheme
     • Additional annual leave above statutory minimum based on service
     • Enhanced family leave (maternity, paternity and adoption leave and pay)
    • Inclusive wellbeing benefits
    • Employee Assistance Programme including free 24/7 independent counselling and occupational health services
     • Learning and development opportunities
     • Free membership to our reward and discount platform
     • Access to Blue Light Card and other NHS Discount Schemes

Due to the nature of this position, employment is subject to proof of eligibility to work in the UK, completion of a satisfactory enhanced DBS disclosure and three years of references.
