In this role, you’ll oversee a busy service supporting safe and timely discharges across a 200-bed pathway. You’ll play a key role in allocating cases, monitoring performance, and ensuring that assessments and reviews are completed within expected timeframes – including 14–28 day assessments and 6–12 week reviews.
This is a hands-on leadership role where you’ll guide and support your team through a light-touch triage system, ensuring workflows are efficient and outcomes remain person-centred. You’ll be instrumental in maintaining high standards of practice while supporting staff with decision-making, prioritisation, and complex cases.
You’ll be working alongside a strong multidisciplinary network, helping to drive effective discharge planning and ensuring individuals are supported to return home safely with the right level of care in place.
