Location: Medway
Rate: £38 per hour (Umbrella)

Working Pattern: Hybrid (3 days office / 2 days home)
Service Care Solutions are recruiting for an Adult Social Worker to join the Targeted Review Team within a local authority in Medway.
Key Responsibilities:
Manage a mixed caseload within a community social work team
Undertake strengths-based assessments and develop person-centred care plans
Carry out face-to-face visits and interventions
Support adults, families, and carers to promote independence and wellbeing
Work in line with the Care Act, Mental Capacity Act, and safeguarding procedures
Collaborate with multi-agency partners to deliver effective outcomes
Maintain accurate case records and prepare professional reportsRequirements:
Qualified Social Worker with Social Work England registration
Completed ASYE
Experience working with adults in a statutory setting
Knowledge of relevant legislation (Care Act, MCA, etc.)
