Job Description
Cromwell Medical Staffing is currently seeking an experienced Band 6 Community Psychiatric Nurse (CPN) to support community mental health services on a 12 -week placement. This is an excellent opportunity for a skilled mental health nurse to manage a defined caseload, deliver high-quality, patient-centred care, and contribute to effective community-based mental health interventions.
Key Responsibilities
* Assess, plan, implement, and evaluate care for a caseload of service users experiencing a range of mental health conditions
* Undertake comprehensive mental health and risk assessments, ensuring appropriate risk management plans are in place
* Act as Care Coordinator in line with CPA (Care Programme Approach) requirements
* Develop and deliver person-centred care plans in collaboration with service users, carers, and the multidisciplinary team
* Participate in the Duty Worker and Initial Assessment rota, providing timely assessments and interventions
* Support safe and effective discharge planning from inpatient services and facilitate smooth transfers of care
* Monitor mental health status, recognise deterioration, and escalate concerns appropriately
* Maintain accurate, timely, and legally compliant clinical documentation
* Work collaboratively with psychiatrists, psychologists, social workers, and other healthcare professionals
* Provide support, education, and guidance to service users and their families
Requirements
* NMC Registered Nurse – Mental Health (RMN)
* Minimum 6 months’ recent experience working as a Community Psychiatric Nurse
* Experience managing caseloads within community mental health services
* Strong risk assessment and care planning skills
* Experience working within CPA framework
* Excellent communication and multidisciplinary teamwork skills
* Right to work in the UK
* Up-to-date mandatory training (PMVA/MAPA desirable, Safeguarding, BLS, Manual Handling)
