    We are seeking an experienced Advanced Practitioner to join a dynamic Children with Disabilities Team in the vibrant area of Southend-on-Sea. This exciting opportunity offers a full-time permanent position with an annual salary ranging from £46,142 to £52,413.     What you will do:

    - Provide efficient and cost-effective services to children, young people, and their families, ensuring professional assessments of their needs.

    - Investigate, evaluate and analyse case information with clarity, resulting in evidence-based conclusions and actionable plans.

    - Develop practice that considers the individual holistically, taking into account their immediate and extended social environment.

    - Ensure all case management complies with statutory requirements and council policies.

    - Prepare and submit written reports, including care plans and statements for court proceedings.

    - Keep the Team Manager informed of any workload issues and seek advice when needed.

    - Maintain accurate case records using ICT, and report on key performance management data.

    - Liaise proactively with other agencies, organisations, parents, and carers.

    - Attend conferences, reviews, and court, presenting council plans with professionalism.

    - Oversee Transition Plans, ensuring robust processes are in place for young people entering adulthood.

    - Supervise Family Support Workers, providing regular guidance and support.

    - Allocate and review direct work undertaken by Family Support Workers, monitoring progress and impact.


    Requirements of this Advanced Practitioner role:

    - A Social Work Qualification or equivalent.

    - Understanding of relevant legislation.

    - Eligibility for registration.

    - Previous experience with Children with Disabilities (CWD).
